Falling in Elderly p2

B. Patient falls has increased ---  % over the past --- years due to the increase in patients disease factors and age-related health issues.

My preceptor thinks that patient falls is a good area for me to focus on because preventing patient falls is an ongoing focus for the facility. It helps create better patient outcomes and lowers facility budget when patients are discharged efficiently with less issues then they came in the facility with. If there is anything that can be added to assist with the prevention of patient falls in the facility, then my preceptor is more then willing to listen to what data or research has proven positive for other patients in long term care facilities. (USE AGENCY DATA HERE : this may include verbal general estimates, see personal communication, APA for how to do this.)

  1. Aggregate-Focused Health Plan
    1. Target Group Needs Assessment Method.

A. Focus group- facility patients who are able and willing to discuss with me their needs and desires when eliminating falls and discussing reasons patients feel they fall.

  1. What do you feel puts you at risk for falls?
  2. What would you like staff members to do for preventing falls that they are not already doing?
  3. Do you use your call light as needed when you are feeling weak, prior to ambulation?
